An exciting opportunity for an experienced Registered Nurse with leadership skills to make a difference!

Opportunity

The Regional Health Lead is responsible for case managing the health needs of Ability WA customers in a specified metro-based region. The current vacancy exists for the South metro region. The responsibilities include providing clinical practice advice and consultancy to the Community Services team to promote optimal health and wellbeing of customers primarily via telepractice and/or direct nursing support for customers with disability related health needs. The successful candidate will continue to practice their clinical skills such as PEG Management, Wound Care, Catheter Management while also having protected office based time for the development of care plans and case management tasks.

 

How you’ll make a difference 

The Regional Health Lead will:

  • Provide clinical support, advice and interventions directly to customers with disability related health needs.
  • Monitor and support the health needs of customers with complex health issues.
  • Coach and provide advice to Community Service (CS) teams, CS Managers, House Coordinators and Support Workers to monitor, maintain and act on customer health and wellbeing issues.
  • Work collaboratively with health practitioners, therapists, families/ guardians and other stakeholders where advocacy and/or a multidisciplinary approach is required to meet a customer’s wellbeing outcomes.
  • Evaluate, develop and plan clinical assessments and support plans to guide Support Worker practice e.g., Bowel care plan
  • Support the Nurse Manager and Quality & Governance Team to develop clinical practice frameworks, governance frameworks and guidance materials.
  • Develop strong collaborative relationships with Coordinators, Managers and the Quality governance team to promote a high standard of clinical practice.
  • Review and provide advice on clinical incidents, hospital admissions and discharges, process and documentation such as medication errors, Annual Medical Reviews and Medication Audits.

About you

What we'll need to help kick-start your journey with us:

  • Current registration in the category of Registered Nurse by the Australian Health Practitioner Regulation Agency (AHPRA).
  • Minimum four years’ experience as a Registered Nurse is essential.
  • Current clinical nursing skills with the ability to provide contemporary nursing supports for customers with complex needs.
  • Coaching and mentoring approach to staff development.
  • Excellent interpersonal and communication and able to consult with customers, their families, teams and a wide range of stakeholders.
  • Strong alignment & commitment to Ability WA Values.
  • Any Relevant Qualification, National Police Clearance no older than 6 months, Current WA Driver’s Licence, NDIS Workers Orientation Module, NDIS Worker Screening check, Current First Aid Certificate (HLTAID010 or HLTAID003 minimum) and COVID-19 Vaccination Certificate
