Community Registered Nurse (Health Lead)

About the role
We’re pleased to offer an internal opportunity for an experienced Registered Nurse to step into the role of Health Lead, supporting the health and wellbeing of Ability WA customers while providing clinical leadership to Community Services teams.
This role combines hands‑on clinical practice with case management, coaching, and clinical governance responsibilities, offering a rewarding mix of direct impact and strategic contribution.
The opportunity
As a Registered Nurse in the Health Lead team, you will support the Clinical Nursing team to meet the health needs of customers with disability‑related and complex health needs, providing clinical advice and consultancy primarily via telepractice and direct nursing support as required.
You will continue to practice key clinical skills (including PEG management, wound care and catheter management), while having protected office‑based time to develop care plans, complete case management tasks and contribute to clinical frameworks.
This is a part‑time role (22.5 hours per week) based at our Coolbinia office, with flexible working arrangements available.
Key responsibilities
In this role, you will:
- Provide clinical advice, support and interventions to customers with disability‑related health needs
- Monitor and support customers with complex or high‑risk health presentations
- Coach and support Community Services teams, Managers, Coordinators and Support Workers
- Collaborate with health professionals, therapists, families and guardians to support customer outcomes
- Develop, review and implement clinical assessments and support plans (e.g. bowel care plans)
- Support the Nurse Manager and Quality & Governance team with clinical practice and governance frameworks
- Review and provide advice on clinical incidents, hospital admissions/discharges, medication management, AMRs and audits
What we’re looking for
You’ll bring:
- Current AHPRA registration as a Registered Nurse
- At least two years’ RN experience
- Strong contemporary clinical skills supporting customers with complex needs
- A coaching, mentoring and collaborative approach
- Strong communication and stakeholder engagement skills
- Alignment with Ability WA values
You will also hold (or be willing to maintain):
- National Police Clearance (within 6 months)
- Driver’s Licence
- NDIS Worker Screening Check & NDIS Worker Orientation Module
- Current First Aid Certificate (HLTAID010 or HLTAID003 minimum)
